First Visit / Insurance Information and Patient Forms

Your First Visit

 

Thank you for choosing TLC Physical Therapy, Inc. for your physical therapy needs. We ask that you come 15 minutes early for paperwork. Please be prepared to fill out patient questionnaire forms before your first appointment. Your first evaluation usually takes around 45 minutes to one hour.

Please bring the following items with you to your first appointment:

  • Proof of insurance

  • Co-pay if your insurance company has one

  • Referral or physician order

Also be prepared for the following:

  • For lower extremity problems please bring shorts.

  • For shoulder, back or neck problems please bring loose fitting tee shirt or tank top.

Did You Know: South Dakota is a direct access state, meaning that if your insurance allows you can see a PT without a referral.

TLC Physical Therapy is now a member of SilverSneakers. The SilverSneakers program is a wellness program through your current MEDICA health insurance supplement which will pay for your monthly membership at the TLC Fitness Center if you are eligible. Please contact your current health insurance company for eligibility requirements. TLC Physical Therapy also participates in the Silver & Fit Program. The Silver & Fit Fitness Program is a wellness program through your current health insurance which will pay for your monthly membership at the TLC Fitness Center if you are eligible. Please contact your current health insurance company for eligibility requirements.

Patient Forms

 

During your first visit you will be asked to complete the Patient Information Sheet which will include your Name, Address, Insurance Information, as well as, your Doctor’s information.  When you arrive you will also be asked to complete a Medical History form electronically called FOTO which stands for Focus On Therapeutic Outcomes.

Please make sure that you remember to check with your insurance company prior to your appointment in regard to needing a Doctor’s order for Physical Therapy as some insurances require you to see your Medical Doctor prior to starting Physical Therapy.

We also ask that you bring your insurance cards and a list of all medications that you are taking to the initial appointment.
